ABSTRACT

RESUMEN Los aneurismas de la arteria comunicante anterior son complejos y requieren de tratamiento endovascular o microquirúrgico según sus características. El uso de enfoques mínimamente invasivos en microcirugía continúa evolucionando. Presentamos el caso de un paciente de 34 años con un aneurisma cerebral no roto de AcoA de cuello ancho, con escala de Glasgow de 15 puntos. Se realizó un abordaje supraorbitario lateral más "clipaje" total del aneurisma usando angio fluoresceína intraoperatoria. Se logró el objetivo con seguridad y eficacia; al mismo tiempo se optimizó el tiempo quirúrgico y hospitalario, maximizando la comodidad del paciente, estética y reanudación a sus actividades tempranas. SUMMARY Anterior communicating artery aneurisms (ACAA) are complex and require endovascular or microsurgery approaches. The use of minimally invasive microsurgery techniques is evolving. We present the case of a 34-yearold patient with a broad neck, non-broken, ACAA with a Glasgow coma scale of 15 points. A lateral supra-orbitary approach with total clipping of the ACAA using intra-operatory fluorescein angiography was performed. The objective was accomplished successfully and safe, optimizing both intra-operative and hospitalization times, thus,maximizing patient comfort, esthetic and prompt return to normal duties. (AU)

ABSTRACT

Resumo Aneurismas da artéria esplênica (AAE) verdadeiros são uma patologia rara, mas potencialmente fatal. São o terceiro aneurisma abdominal mais comum, após aneurismas da aorta e da artéria ilíaca, e representam quase todos os aneurismas de artérias viscerais. Os aneurismas verdadeiros são responsáveis ​​por 60% dos AAEs e afetam as mulheres quatro vezes mais do que os homens, geralmente relacionados a uma descoberta incidental ou sintomática aumentada que coincide com o uso da ultrassonografia na gravidez. Em pacientes grávidas, a mortalidade, após a ruptura, é de 65-75%, com mais de 90% de mortalidade fetal. Têm múltiplas etiologias, e acredita-se que as influências hormonais e as alterações do fluxo portal durante a gestação desempenhem um papel importante no desenvolvimento do AAE. Esta revisão discorrerá sobre sua história, epidemiologia, fisiopatologia, diagnóstico, e as técnicas atuais de tratamento.


Abstract True splenic artery aneurysms (SAA) are a rare, but potentially fatal, pathology. They are the third most common type of abdominal aneurysm, after aneurysms of the aorta and of the iliac artery, and account for almost the all aneurysms of visceral arteries. True aneurysms account for 60% of SAA and affect four times as many women as men, generally related to increased incidental or symptomatic findings that coincide with use of ultrasonography in pregnancy. Among pregnant patients, mortality after rupture is 65-75%, with fetal mortality exceeding 90%. There are multiple etiologies and it is believed that hormonal influences and changes in portal flow during gestation play an important role in development of SAA. This review discusses their history, epidemiology, pathophysiology, and diagnosis and current treatment techniques.

ABSTRACT

Resumo O aneurisma da artéria torácica interna é uma entidade rara, com apresentação variável e risco potencial de ruptura e de morte. A angiotomografia é o exame diagnóstico de escolha, sendo útil para o planejamento terapêutico. Considerando morbidade do acesso torácico para abordagem direta e o risco imprevisível de ruptura, o procedimento endovascular se apresenta como modalidade terapêutica de escolha para tratamento desse tipo de aneurisma. Descrevemos um caso de aneurisma de artéria torácica interna, com descoberta incidental na investigação de síncope tratado com embolização com micromolas de baixo perfil e de liberação controlada.


Abstract Aneurysm of the internal thoracic artery is a rare entity, with variable presentation and a potential risk of fatal rupture. Angiotomography is the diagnostic test of choice and is useful for planning treatment. Considering the morbidity of thoracic access for a direct approach and the unpredictable risk of rupture, an endovascular procedure is the treatment modality of choice for this type of aneurysm. We describe the case of an internal thoracic artery aneurysm discovered incidentally during investigation of syncope and treated by embolization with low-profile and controlled-release microcoils.

ABSTRACT

A compressão do tronco celíaco pelo ligamento arqueado mediano do diafragma pode causar sintomas inespecíficos como dor abdominal, vômitos e emagrecimento. Existe uma associação comprovada entre estenoses ou oclusões do tronco celíaco e aneurismas da artéria pancreatoduodenal. Nas situações em que essa associação ocorre, a estratégia de tratamento deve ser individualizada. Relatamos o caso de uma paciente com aneurisma de artéria pancreatoduodenal associado à compressão do tronco celíaco pelo ligamento arqueado, manejados, respectivamente, por técnicas endovasculares e laparoscópicas


Compression of the celiac axis by the median arcuate ligament of the diaphragm can cause nonspecific symptoms such as abdominal pain, vomiting, and weight loss. There is a known association between stenosis or occlusion of the celiac trunk and aneurysms of the pancreaticoduodenal artery. Treatment strategies for patients who have this association should be selected on a case-by-case basis. We describe the case of a patient with pancreaticoduodenal artery aneurysm associated with compression of the celiac trunk by the arcuate ligament, which were managed with endovascular and laparoscopic techniques, respectively

ABSTRACT

PURPOSE: Contrast-induced acute kidney injury (CI-AKI) is associated with poor outcomes after percutaneous coronary intervention. However, CI-AKI has rarely been evaluated within the neurovascular field. The aim of this study was to investigate the incidence and clinical implication of CI-AKI after coil embolization in patients with an aneurysmal subarachnoid hemorrhage (aSAH). MATERIALS AND METHODS: Between January 2005 and March 2016, 192 patients who underwent coil embolization were enrolled in this study. CI-AKI was defined as an increase from baseline serum creatinine concentration of >25% or >0.5 mg/dL within 72 hours after coil embolization. A poor clinical outcome was defined as a score of ≥3 on the modified Rankin Scale at one-year post-treatment. RESULTS: A total of 16 patients (8.3%) died as a result of medical problems within one year. CI-AKI was identified in 14 patients (7.3%). Prominent risk factors for one-year mortality included CI-AKI [odds ratio (OR): 16.856; 95% confidence interval (CI): 3.437–82.664] and an initial Glasgow Coma Scale (GCS) score ≤8 (OR: 5.565; 95% CI: 1.703–18.184). A poor clinical outcome was associated with old age (≥65 years) (OR: 7.921; 95% CI: 2.977–21.076), CI-AKI (OR: 11.281; 95% CI: 2.138–59.525), an initial GCS score ≤8 (OR 31.02; 95% CI, 10.669–90.187), and a ruptured aneurysm (p=0.016, OR: 4.278) in posterior circulation. CONCLUSION: CI-AKI seems to be an independent predictor of the overall outcomes of aSAH after endovascular treatment.

ABSTRACT

Aneurismas de artéria poplítea (AAPs) correspondem a 70,00% dos aneurismas periféricos. A indicação cirúrgica é para aneurismas com diâmetros maiores que 2,0 cm ou sintomáticos. O tratamento é feito por técnicas cirúrgicas convencionais ou endovasculares. Esta última tem ganho muitos adeptos, mas ainda não há consenso estabelecido sobre sua indicação. Objetivo Apresentar a experiência da Divisão de Cirurgia Vascular e Endovascular do Hospital das Clínicas de Ribeirão Preto da Universidade de São Paulo no tratamento dos AAPs. Método Foram revisados casos de reparo convencional e endovascular de AAPs tratados nos últimos cinco anos, avaliando dados demográficos, comorbidades, indicação cirúrgica, complicações pré e pós-operatórias precoces e tardias, tempo de internação e de perviedade em até um ano. Resultados Foram realizadas no período dez cirurgias endovasculares (CE) e 21 cirurgias abertas (CA). O grupo CE teve maior frequência de comorbidades. Houve maior frequência de pacientes sintomáticos no grupo CA (85,00%) do que no grupo CE (40,00%). O Grupo CE apresentou menor número de complicações clínicas e cirúrgicas. A idade entre os grupos e o tempo de internação de cada grupo não apresentaram diferença estatística. A perviedade primária em um ano no Grupo CE foi de 80,00%, enquanto no Grupo CA foi de 75,00%. Conclusão O tratamento endovascular para AAPs apresenta bons resultados, em termos de perviedade com taxas de complicações aceitáveis, em pacientes com risco cirúrgico elevado e anatomia favorável, justificando, assim, a necessidade de mais estudos controlados para modificar a posição da técnica endovascular como uma terapia alternativa para casos selecionados.


Popliteal artery aneurysms (PAAs) account for 70% of peripheral aneurysms. Surgery is indicated for aneurysms that have diameters greater than 2.0 cm or are symptomatic. Repair can be achieved by conventional surgical techniques or using endovascular methods, which are becoming increasingly popular, but for which there is not yet a consensus on indications. Objective To describe the experience of treating PAAs at the vascular and endovascular surgery department of the Hospital das Clínicas de Ribeirão Preto, affiliated to the Universidade de São Paulo (Brazil). Method A review was conducted of cases of conventional and endovascular repair of PAAs over the last 5 years, analyzing demographic data, comorbidities, surgical indications, preoperative and early and late postoperative complications, length of hospital stay and patency, during follow-up of up to 1 year. Results During the period analyzed, ten endovascular surgeries (ES) and 21 open surgeries (OS) were performed. The ES group exhibited a higher frequency of comorbidities. There was a higher frequency of symptomatic patients in the OS group (85%) than in the ES group (40%). The ES group exhibited a lower number of clinical and surgical complications. There were no statistical differences between the groups in terms of age or length of hospital stay. Primary patency at 1 year was 80% in the ES group and 75% in the OS group. Conclusions Endovascular treatment for PAAs offers good results in terms of patency, with acceptable complication rates, in patients with high surgical risk and favorable anatomy. Controlled studies are therefore warranted to validate the endovascular technique and afford it the status of an alternative procedure for use in selected cases.

Introdução: Com os recentes avanços nas técnicas endovasculares e com o surgimento de endopróteses mais flexíveis, o tratamento das lesões aneurismáticas da artéria poplítea tem se tornado mais frequente. O objetivo desse estudo foi avaliar os desfechos clínicos a curto e médio prazos do tratamento de lesões aneurismáticas da artéria poplítea com o uso de endopróteses flexíveis. Métodos: Estudo retrospectivo, longitudinal, realizado em dois centros, no período de janeiro de 2011 a fevereiro de 2014. Foram avaliados características populacionais, dados do procedimento e imagens radiológicas no seguimento médio de 1 ano, sendo obtidas as taxas de morbimortalidade, complicações e perviedade da endoprótese. Resultados: Treze pacientes do sexo masculino, com idade de 66 ± 9 anos, foram submetidos ao tratamento de aneurismas de artéria poplítea em 15 membros. Na avaliação do leito de deságue, a maior parte dos pacientes possuía pelo menos duas artérias da perna pérvias (92,3%). O implante do stent ocorreu no segmento médio em 57,1% e, no segmento distal da artéria poplítea, em 42,9% dos procedimentos. Foi possível realizar a revascularização da lesão-alvo em todos os casos, sendo que, em quatro membros, foi necessário o uso de dois stents. Foram utilizados 17 stents Viabahn® e 2 stents Multilayer®. Durante o seguimento de 12 meses, não ocorreram fraturas de stents. A taxa de perviedade primária foi de 53,3% e a de salvamento de membro de 100%. Background: With the recent advances in endovascular techniques and the emergence of more flexible endoprosthesis, the treatment of popliteal artery aneurysms has become more frequent. The objective of this study was to evaluate the short and mid-term clinical outcomes of the treatment of popliteal artery aneurysms with the use of a flexible endoprosthesis. Methods: Retrospective longitudinal study conducted in two sites from January of 2011 to February of 2014. Populational characteristics, procedure-related data, and radiologic imaging were evaluated at a mean follow-up of 1 year. Morbidity and mortality rates, complication rates, and stent patency rates were obtained. Results: A total of 13 male patients, mean age 66 ± 9 years, were submitted to popliteal artery aneurysm treatment in 15 limbs. Run-off vessel evaluation showed that most patients had at least two patent arteries in the legs (92.3%). Stents were implanted in the middle segment in 57.1% and in the distal segment of the popliteal artery in 42.9% of the procedures. Target lesion revascularization was achieved in all of the cases and two stents were required in four limbs. Seventeen ViabahnTM and two MultilayerTM stents were used. During the 12-month follow-up there were no stent fractures. The primary patency rate was 53.3% and limb salvage rate was 100%. Introdução: Os aneurismas da artéria renal são raros e constituem um desafio ao tratamento endovascular. Nosso objetivo foi descrever e analisar as técnicas e táticas no tratamento endovascular do aneurisma da artéria renal, verificando os resultados a curto e médio prazos de uma série consecutiva de casos. Métodos: Estudo retrospectivo, de procedimentos realizados no período de janeiro de 2010 a dezembro de 2013, em que foram analisados: o sucesso técnico e terapêutico, a morbimortalidade, e a taxa de vazamentos e de reintervenções. Resultados: Em um total de seis pacientes tratados, a idade média foi de 41 ± 5 anos e todos eram do sexo feminino. A maioria apresentou aneurismas saculares tipo II (83,3%). Foram utilizadas técnicas de remodelamento com uso de stent e molas em quatro casos, embolização segmentar renal em um caso e tratamento com endoprótese Multilayer® em outro. O sucesso técnico e terapêutico foi de 100 e 83,3%, respectivamente. Em um paciente, houve isquemia de polo superior renal, que evoluiu para hematúria e dor incontrolável, necessitando de nefrectomia. Não ocorreram óbitos e nem oclusão das artéria renais nativas e de seus ramos durante o acompanhamento de 1 ano. Conclusões: O tratamento endovascular do aneurisma de artéria renal demonstrou ser uma alternativa viável à cirurgia convencional com baixa morbidade. O estudo detalhado da vascularização renal e da localização do aneurisma determina a escolha da técnica endovascular a ser utilizada. O aneurisma da artéria renal do tipo II foi a morfologia mais frequentemente encontrada e pode ser tratado com sucesso por técnicas de remodelamento com o uso de stent e mola.


Background: Renal artery aneurysms are rare and constitute a challenge to endovascular treatment. Our objective was to describe and analyze the techniques and strategies for the endovascular treatment of renal artery aneurysms verifying short and medium-term results in a consecutive series of cases. Methods: Retrospective study of procedures performed from January 2010 to December 2013, analyzing technical and therapeutic success, morbidity and mortality, the rate of endoleaks and reinterventions. Results: In a total of six patients treated, mean age was 41± 5 years and all patients were female. The majority of the patients had type 2 saccular aneurysms (83.3%). Remodeling techniques using stent and coils were used in four cases, embolization of renal polar branch was used in one case and treatment with a Multilayer® endoprosthesis in another case. Technical and therapeutic success rates were 100% and 83.3%, respectively. In one patient there was upper renal pole ischemia, which progressed to uncontrollable hematuria and pain, requiring nephrectomy. There were no deaths or occlusion of the native renal artery and its branches during the 1-year follow-up. Conclusions: Endovascular treatment of renal artery aneurysm proved to be a feasible alternative to conventional surgery with low morbidity. A detailed study of renal vasculature and aneurysm location determines the choice of the endovascular technique to be used. Type II renal artery aneurysm was the most frequent morphology observed and may be successfully treated by remodeling techniques using stents and coils.

ABSTRACT

We report a 25yearold mole admitted to a critical care unit for fever, lung opacities and acute respiratory failure. A chest angio-CAT sean showed multiple pulmonary artery aneurysms. A deep venous thrombosis of both lower limbs was also documented. A Hughes-Stovin syndrome was postulated. An inferior vena cava filter was placed. The patient received antimicrobial therapy subsiding fever and respiratory failure. Subsequently, he was treated with intravenous and oral steroids and one dose of cyclophosphamide. The patient was discharged in good conditions fifteen days after admission.

ABSTRACT

Splenic artery aneurysms are the most common visceral aneurysm. They are usually asymptomatic, but may present rarely as abdominal pain, or with symptoms and signs of acute rupture. This is a case report of a 42 years old female patient who presented with mild abdominal pain and was found to have a splenic artery aneurysm with a challenging anatomy of the prominent splenic artery. The patient refused the surgical option of treatment, and was treated by endovascular embolization, using a double catheter technique.

ABSTRACT

Relatamos caso de um paciente de 14 anos, sexo masculino, que foi admitido com queixa de embaçamento visual bilateral há dois anos. Ao exame oftalmológico observou-se leve hiperemia e edema de disco óptico bilateral, exsudação retiniana, poucas hemorragias retinianas, múltiplos aneurismas, assim como sinais de vasculite. A angiofluoresceinografia demonstrou isquemia periférica extensa, dilatações e hiperfluorescência das paredes dos vasos, e vazamento tardio do disco óptico nas fases finais do exame em ambos os olhos. Este caso representa uma rara entidade caracterizada por oclusão retiniana vascular periférica, vasculite retiniana, múltiplos aneurismas retinianos e neurorretinite (IRVAN). Avaliação sistêmica e laboratorial não revelaram nenhuma anormalidade. O paciente foi submetido à panfotocoagulação de retina com laser de argônio em ambos os olhos, e iniciado tratamento com prednisona via oral, com manutenção da acuidade visual de 20/25 depois de um ano de acompanhamento. O tratamento com laser deve ser considerado quando houver qualquer evidência angiográfica de má perfusão retiniana, e antes do desenvolvimento de qualquer sinal de neovascularização de retina.


A case of a 14 year-old boy that was admitted complaining of bilateral ocular visual blurring for 2 years is reported. The ophthalmological examination disclosed bilateral mild optic disc hyperemia and swelling, retinal exudation, few retinal hemorrhages, multiple aneurysms, as well as vasculitis. Fluorescein angiography showed extensive peripheral retinal ischemia, dilatations and hyperfluorescence of the vessels walls, and leakage of the optic disc in the late phases in both eyes. This rare case represents an entity characterized by peripheral retinal vascular occlusion, retinal vasculitis, multiple posterior retinal aneurysms, and neuroretinitis (IRVAN). Systemic evaluation and laboratory work-up did not suggest any systemic abnormality. Panretinal laser photocoagulation was performed in both eyes, and the patient was treated with oral prednisone with maintenance of the visual acuity after 1 year of follow-up. Laser treatment should be considered when angiographic evidence of widespread retinal no perfusion is present, and before the development of signs of retinal neovascularization.

ABSTRACT

O tratamento endovascular dos aneurismas de artéria renal tem sido descrito como alternativa à cirurgia convencional. Relatamos o caso de uma paciente com um aneurisma de artéria renal complexo à direita que apresentava hipertensão arterial de difícil controle. O tratamento endovascular foi realizado com a técnica de remodelagem de colo (técnica de Moret), ou técnica de embolização assistida por balão. A paciente obteve normalização da pressão arterial após o procedimento sem recidiva dos sintomas ou necessidade do uso de drogas anti-hipertensivas.


Endovascular treatment of renal artery aneurysms has been described as an alternative to conventional surgery. We report the case of a patient with complex renal artery aneurysm on the right kidney who had hard-to-control arterial hypertension. Endovascular treatment was performed with the aneurysm neck remodeling technique (Moret technique), or balloon-assisted coil embolization. The patient achieved blood pressure normalization after the procedure without recurrence of symptoms or need for antihypertensive drugs.

OBJETIVO: Relatar os resultados da aplicação de técnicas endovasculares no tratamento de aneurismas cirsoideos do couro cabeludo. MATERIAIS E MÉTODOS: Quatro pacientes com diagnóstico de aneurismas cirsoideos foram submetidos ao tratamento por via endovascular. Todos os quatro pacientes incluídos nesta série tinham malformações arteriovenosas e foram tratados apenas com embolização. RESULTADOS: Três pacientes foram submetidos a tratamento endovascular mediante embolização transarterial e um foi tratado por punção direta da porção venosa. Os resultados clínicos e cosméticos foram satisfatórios em todos os pacientes. Não houve recidiva clínica durante o período de acompanhamento. CONCLUSÃO: A via endovascular é uma alternativa segura e eficaz no tratamento dos aneurismas cirsoideos. Embora possa ser efetivamente utilizado como uma alternativa adjuvante ou complementar à cirurgia, especialmente quando é necessário lidar com aferências profundas, a maioria dos casos pode ser totalmente curada apenas com a terapêutica endovascular. A escolha do método de tratamento deve ser baseada em uma variedade de características próprias da lesão, incluindo sua angioarquitetura, tamanho e apresentação clínica.


OBJECTIVE: To report results of the application of endovascular techniques in the management of scalp cirsoid aneurysms. MATERIALS AND METHODS: Four patients diagnosed with cirsoid aneurysms were submitted to treatment by endovascular approach. All the four patients included in the present series had arteriovenous malformations and were treated solely by embolization. RESULTS: Three of the patients underwent endovascular treatment by transarterial embolization and one was treated by direct puncture of the venous segment. Both clinical and cosmetic outcomes were satisfactory in all of the patients. Clinical relapse was not observed along the follow-up period. CONCLUSION: The endovascular approach is safe and effective in the management of crisoid aneurysms. Although this technique can be used as an adjuvant or complement to surgery, particularly in cases where deep afferents are involved, complete resolution can be achieved only with endovascular treatment. The choice of treatment method should be based on a range of typical characteristics of the lesion, including angioarchitecture, size and clinical presentation.

ABSTRACT

Previous studies on aneurysms in Nigeria have either been case reports or studies of peripheral aneurysms. No study has comprehensively evaluated all aspects of this disease as an entity among Nigerians. The need therefore arises to re-evaluate this lesion so as to make deductions on incidence; sex ratio; aetiology and management. This is a retrospective descriptive study of arterial aneurysms at the University of Nigeria Teaching Hospital (UNTH); Enugu. Those treated between January 1993 and December 2002 were included for the study. Datawere obtained frommedical records for all patients admittedwith aneurysms over the study period. Atotal of 26 patientswere admitted during the period; but 24 case noteswere analysed. The age rangewas 10-75 yearswithmale: female ratio of 1.4:1. Traumatic pseudo aneurysms accounted for 16 cases (67.0)while true aneurysms comprised the remaining.Of all the aneurysms; femoral artery with most of the pseudo aneurysms had 8 out of 24 (33.0).This is followed by the infra-renal abdominal aorta (5/24; 21.0) and other sites (11/24; 46.0). Diagnoses were made clinically in most cases and by abdominal ultrasonography in abdominal aortic aneurysms. Twenty-one patients had surgical interventionwith 9.5operativemortality. The incidence of aneurysm is low in our locality (2.6/year) based on the rate of diagnosis.Abdominal aorta harbours most of the true aneurysms with diameter ranging from 8.0-15.0cm without rupture. Untreated; all will eventually rupture with catastrophic consequences. Treatment involved excision with graft interposition. This is not only expensive but the graft is often not readily available. As a solution; grafts should be stocked with drug revolving fund

ABSTRACT

Therapeutic embolization is defined as the voluntary occlusion of one or several vessels, and this is achieved by inserting material into the lumen to obtain transient or permanent thrombosis in the downstream vascular bed. There are a number of indications for this approach in urological practice, in particular for the patients with parenchymatous or vascular kidney disease. In this review, we present the different embolization techniques and the principally employed occluding agents, and then we present the principal clinical indications and we discuss other pathologies that may benefit from this non-invasive therapy. The complications, side effects and main precautions associated with this approach are also described.

ABSTRACT

El pseudoaneurisma femoral (PAF) es una complicación de las intervenciones percutáneas, con incidencia entre 0,5% y 2%. El diagnóstico es clínico y se establece con imágenes de ultrasonido-Dopper color de la arteria femoral con numerosas alternativas terapéuticas para su resolución. Describir la conducta terapéutica por medio de la inyección de trombina guiada por ultra sonido y la experiencia del Centro Cardiovascular La Floresta. El diagnóstico de PAF se realizó en base a la manifestaciones clínicas y hallazgos por ultrasonido-Doppler a color. Seguidamente se accedión al saco del PAF con una aguja de punción lumbar 22G, guiada por ultrasonido y se inyectó lentamente trombina heteróloga de origen bovino, tomando nota de las dosis utilizadas hasta lograr la trombosis del saco. Se realizó seguimiento clínico y ultrasonográfico a las 24,48 horas, 3 y 6 meses. Se trateron 15 pacientes consecutivos con edad promedio 62 ± 5 años entre enero de 2002 y noviembre de 2008. En todos los pacientes se logró la trombosis del PAF con la primera inyección con mejoría clínica dentro de las primeras 24 horas. Once pacientes (79%) fueron seguidos clínicamente y ecográficamente a las 24 - 48 horas, y clínicamente a los 3 y 6 meses. La inyección de trombina guiada por ultrasonido puede ser considerada para el tratamiento del PAF, ya que demostrado ser un procedimiento mínimamente invasivo, seguro y de bajo costo.


Femoral pseudoaneurysm (FPA) is a complication of percutaneous interventions, with an incidence of 0.5%-2%. The diagnosis is clinical and is usually confirmed by ultrasound-color Doppler (UCD) of the femoral artery with multiple therapeutic options. To describe the therapeutic approach to seal the FPA with the use of thrombin guided by ultrasound and to analize the experience at the Centro Cardiovascular La Floresta. The diagnosis of EPA was made on clinical basis and UCD. The sac of the EPA was accessed with a 22G needle under ultrasoun guidance followed by careful heterologous bovine thrombin infection until thrombosis of the sac was abtained. Clinical and UCD follow-up was made at 24,48 hours and at 3 and 6 months. Fifteen consecutive patients, 65 ± years, between january 2002 and november 2008 were treated. In all patients thrombosis of the sac of the FPA was obtained with the first injection and clinical improvement was noted within the first 24 hours. Eleven patients were followed-up clinically and with UCD at 24 and 48 hours and clinically at 3 and 6 months. Thrombin injection with ultrasound guidance may be considered as a method to treat EPA, since it has shown to be minimally invasive, safe and at low cost.

Antecedentes: Hasta el a±o 2006 el único tratamiento del pseudoaneurisma femoral post cateterismo en el INCOR, era la reparación quirúrgica. Se necesitade un tratamiento no invasivo que acorte la estancia hospitalaria y la necesidad de tratamientos quirúrgicos,lo que representaría menores costos, menor estancia hospitalaria, menos riesgo de complicaciones por la cirugía, como infecciones y sangrado. Objetivo: Determinar el porcentaje de éxito del cierre de Pseudoaneurisma femoral post cateterismo por compresión con Guía Ecogrßfica, en el INCOR periodo Enero 2006 a Enero 2007. Procedimientos: Todos los pacientes en los que se sospecha de pseudoaneurisma post cateterismo del INCOR, son remitidos al laboratorio de Ecografía. Si se confirma el diagnostico y no tiene criterios de exclusión que son: necesidad de tratamiento quirúrgico por necrosis de piel, infección, expansión rßpida, síndrome de compresión, lo ingresamos al estudio, previo consentimiento informado, realizßndose la compresión con guía ecogrßfica con premedicaciónen los casos que sea necesaria usando morfina, atropina y una vía periférica permeable. Si el procedimiento es exitoso, se harß un control en las primeras 24 horas, y si no hay recidivas, serß dado de alta. Los procedimientos no exitosos serßn derivados a cirugíavascular. Resultados: Se diagnosticaron 16 casos de pseudoaneurisma post cateterismo durante el 2007, el promedio de edad fue 61,37a±os rango 42 - 81 a±os, sexo masculino 56 por ciento y femenino 44 por ciento. El tipode procedimiento fue 11(69 por ciento) para cateterismos diagnósticos, 5 (31 por ciento) para cateterismos terapéuticos. El lugar de punción y localización previo al diagnóstico del pseudoaneurisma se encontró en femoral superficial 9(56 por ciento) y femoral común 7 (44 por ciento). CONTEXTO: Dos aneurismas periféricos, o da artéria poplítea é o mais freqüente, correspondendo a 70 a 80 por cento dos casos. O tratamento cirúrgico convencional consta de exclusão do aneurisma e interposição de enxerto em ponte ou de ressecção parcial ou total do aneurisma e reconstrução arterial com enxerto em continuidade. O tratamento endovascular surgiu como uma alternativa ao reparo convencional. OBJETIVO: Avaliar o uso de endoprótese para o tratamento endovascular do aneurisma de artéria poplítea. METODOLOGIA: Num total de 17 pacientes, todos do sexo masculino, 11 foram tratados por técnica endovascular, utilizando-se próteses Hemobahn e Viabahn. RESULTADOS: Um paciente apresentou pseudo-aneurisma no pós-operatório imediato. Dentre as complicações tardias, um paciente apresentou endoleak distal da prótese após 7 meses, e houve oclusão da endoprótese em outro. Os nove pacientes restantes apresentaram controle de eco-Doppler satisfatório aos 20 meses, resultando em uma perviedade primária de 90 por cento em um período médio de 27 meses de seguimento. CONCLUSÃO: O tratamento endovascular para aneurisma de artéria poplítea é factível e apresenta algumas vantagens em relação ao tratamento aberto, como menor tempo de internação e de recuperação.


BACKGROUND: The most commonly occurring aneurysms in the periphery are those involving the popliteal artery. They comprise 70-80 percent of all such aneurysms. Conventional aneurysm repair consists of either opening the aneurysm sac and interposing a bypass graft or aneurysm ligation combined with bypass grafting. Endovascular treatment is an alternative to conventional repair. OBJECTIVE: To evaluate use of stent graft in the endovascular treatment of popliteal artery aneurysms. METHODS: We analyzed 17 male patients; of these, 11 were treated with endovascular stent graft, using Hemobahn and Viabahn stent grafts. RESULTS: One patient had pseudoaneurysm in the immediate postoperative period. Among late complications, one patient had distal endoleak after 7 months, and there was stent graft occlusion in another patient. The remaining nine patients had satisfactory Doppler ultrasonography control at 20 months, which resulted in a primary patency of 90 percent over mean follow-up time of 27 months. CONCLUSION: Endovascular repair of a popliteal artery aneurysm is feasible and has some advantages compared with the open treatment, such as shorter hospital stay and recovery.

ABSTRACT

O aneurisma de artéria poplítea é relativamente raro, porém representa cerca de 85 por cento de todos os aneurismas arteriais periféricos. Apresenta-se geralmente com um quadro de complicação isquêmica e elevado risco de perda do membro acometido. Em função disso, preconiza-se seu tratamento eletivo, atualmente realizado com resultados satisfatórios pela técnica endovascular. Relatamos nossa experiência com a utilização do stent de nitinol auto-expansível revestido com PTFEe - Fluency (Bard, Alemanha), reforçado internamente com o stent de nitinol auto-expansível Zilver (Cook, EUA) no tratamento de um aneurisma de artéria poplítea.


Popliteal artery aneurysm is relatively rare, but represents around 85 percent of all peripheral arterial aneurysms. It is usually presented with ischemic complication and high risk of limb loss. For that reason, its elective treatment is indicated and currently carried through with satisfactory results using endovascular techniques. We describe our experience with the use of an ePTFE-covered nitinol self-expandable stent graft - Fluency (Bard, Germany), reinforced internally with the nitinol self-expandable Zilver stent (Cook, USA) for the treatment of a popliteal artery aneurism.
